The picture refers to the character of Atlas in Greek mythology. According to mythology, Atlas has to carry the sky on his shoulders forever as punishment for rebelling against the gods. Later, he is seen carrying the world on his shoulders in popular works of art.
It is believed that Trump wanted to convey through this picture that he is also carrying the responsibility of the entire world on his shoulders alone. However, he did not give any caption with the picture.
